Fast Pass Checklist
The following items must be completed and returned to your home school’s performing arts office, no later than July 4th, to qualify for a Fast Pass at Band Camp Registration.
Note: You can scan & email completed forms to your school’s PA Admin Assistant, rather than turning them in at rehearsal. LC students should email completed forms to Tabatha Isaacs at email@example.com LN families should email completed forms to Jen Jansen at firstname.lastname@example.org
1.) Performer Contract
2.) Medical Consent & Authorization Form
4.) IHSAA Physical – ALL sections must be completed, including the physician’s examination on page 3. (Note: Physician’s Exam must have been completed AFTER April 1, 2021, per IHSAA regulations)
6.) Payments Made – For Fast Pass Eligibility, a minimum of $300 paid no later than Friday, July 16th, to allow time for processing. See the Financial Information section below, for more information. If you would like to request a payment plan different from the plan outlined on the Financial Information form, please contact your home school’s performing arts admin assistant.
2021 Financial Information
All payments will be made through your home school’s booster organization. The total cost for MPLT 2021 is $600, which includes all fees (except for the cost of shoes for new marchers).
